About once a year for a typical household, and more often for large families, holiday lets or anywhere with heavy use. Sewage treatment plants are normally de-sludged every 6 to 12 months. Leaving it too long pushes solids into the drainage field, which is a far more expensive repair.
Only if it discharges to a watercourse — a ditch, stream or surface water drain. Those systems must be replaced with a sewage treatment plant or redirected to a proper drainage field, and it is commonly picked up when a property is sold.
